Zusammenfassung
Nierenzellkarzinome sind mit steigender Inzidenz die häufigsten malignen Tumorerkrankungen der Nieren. Die histomorphologische und molekularpathologische Klassifizierung der Nierenzellkarzinome ist für die klinische Prognoseabschätzung und Therapiewahl von zentraler Bedeutung. Die wichtigsten und häufigsten Subtypen sind die klarzelligen, die papillären und die chromophoben Nierenzellkarzinome. Entstehung und Progression der klarzelligen Nierenzellkarzinome werden bedingt durch den Wirkungsverlust des VHL-Tumorsuppressorgens, die intrazelluläre Akkumulation von HIF-1, die Aktivierung des AKT-mTOR-Signalwegs sowie die Deregulation von Tyrosinkinaserezeptoren. Diese molekularen Aberrationen sind Grundlage für neue zielgerichtete Therapieansätze, wie z. B. mTOR- oder Tyrosinkinaseinhibitoren. Eine entscheidende Herausforderung für die Zukunft besteht in der Identifizierung von prädiktiven Markern, welche das Ansprechen von Patienten mit Nierenzellkarzinomen auf diese neuen Therapieformen vorhersagen können.
Abstract
Renal cell carcinomas are the most frequent malignant tumors of the kidneys. The histomorphology and molecular pathology play pivotal roles in the assessment of the clinical prognosis and patient stratification. The most important subtypes of renal cell tumors are clear-cell, papillary and chromophobe renal cell carcinomas. The development and progression of clear-cell renal cell carcinomas depend on the loss of the tumor suppressor VHL, intracellular accumulation of HIF-1, activation of the AKT-mTOR pathway and deregulation of tyrosine kinase receptors. These molecular aberrations are targeted by novel therapy strategies, such as inhibitors of mTOR or tyrosine kinases. A major future challenge is the identification of biomarkers which predict the response of renal cell carcinomas to novel targeted therapies.
