Zusammenfassung
Hintergrund
Nicht-übertragbare Erkrankungen sind eine der drängendsten Herausforderungen für das Gesundheitswesen und die Gesundheitspolitik. Daher ist die Prävention der wichtigsten Risikofaktoren, allen voran Adipositas, ein zentrales Public-Health-Desiderat. Erfolg versprechen dabei Konzepte, die nicht allein das individuelle Verhalten, sondern die Lebensverhältnisse adressieren. In Deutschland fehlt derzeit aber eine koordinierte, umfassende und vernetzte Präventionsstrategie.
Fragestellung
Die Deutsche Allianz gegen Nichtübertragbare Krankheiten hat vier verhältnispräventive Maßnahmen gegen Adipositas bei Kindern und Jugendlichen vorgeschlagen: täglich 1 h Sportunterricht, Steuern zur Lenkung der Nahrungsmittelpreise, bessere Verpflegung in Schulen und Kindertagesstätten sowie die Beschränkung zielgerichteter Lebensmittelwerbung. Inwieweit dieses Programm zur Entwicklung eines koordinierten Präventionskonzepts beitragen kann, wird in dieser Arbeit geprüft.
Material und Methoden
Die vorgeschlagenen Maßnahmen werden vor dem Hintergrund aktueller wissenschaftlicher Erkenntnisse aus gesundheitspolitischer und sozialethischer Perspektive diskutiert.
Ergebnisse
Alle vier Maßnahmen sind plausibel und können in unterschiedlichem Grade wissenschaftlich unterfüttert werden. Bei der weiteren Ausarbeitung des Programms ist v. a. darauf zu achten, dass die vier Interventionen durch Maßnahmen flankiert werden, die entweder adverse Effekte verhindern oder die langfristige Wirksamkeit sicherstellen. Darüber hinaus bedürfen übergreifende Aspekte größerer Aufmerksamkeit: Probleme durch die föderale Struktur der Bundesrepublik, die Kommunikation zwischen Wissenschaft, Politik und Öffentlichkeit, sowie die Rolle wissenschaftlicher Evidenzen in politischen Debatten.
Abstract
Background
Noncommunicable diseases are challenging health care systems and health policies on a global level. Hence, prevention of core risk factors, especially obesity, is a pressing concern of public health efforts. In general, concepts addressing environmental factors—and not mainly individual behavior—are most promising. Nevertheless, such a coordinated and comprehensive strategy of prevention is currently lacking in Germany.
Objectives
Recently, the German Alliance against Noncommunicable Diseases suggested four political measures to prevent obesity in children and adolescents: one hour of physical activity per day, food taxes, improvement of nutritional quality of diets in schools and nursery schools, and restriction of food advertising. The present paper deals with the question to what extent this program is suitable to contribute to a coordinated and comprehensive strategy of prevention.
Methods
The proposed program is evaluated from a social ethics and health policy point of view with relation to the scientific state of the art.
Results
The four measures are promising and can be substantiated by findings and results of current research, at least to some extent. When the program is considered and developed further, it should be complemented by measures which inhibit unintended adverse effects and ensure long-term efficacy. In addition, more attention has to be paid to some overarching aspects: problems occurring due to the federal system, communication between science, politics and the public, and the function of scientific evidence in policy discourse.
