Zusammenfassung
Ziel dieses Beitrags ist die Untersuchung der Frage, warum im deutschen Sozialversicherungssystem Unterschiede in der Mundgesundheit zwischen den Sozialschichten weiterhin bestehen. Die Einführung des § 21 SGB V, „Gruppenprophylaxe“, 1988 hat zwar den Mundgesundheitszustand insgesamt verbessert, die sozial ungleiche Verteilung von Erkrankungen jedoch nicht. Es wird davon ausgegangen, dass Gesundheit und Krankheit aus einem multifaktoriellen Geschehen entstehen und weitgehend psychosozialen Einflüssen unterliegen, unabhängig vom Versorgungssystem. Hier wurden relevante Studien und deren Ergebnisse aus der internationalen Literatur im Grenzbereich zwischen medizinischer Psycho- und Soziologie und Zahnmedizin analysiert und es wurde dadurch einheitlich dokumentiert, dass überall eine Beziehung zwischen sozialer Schicht, Milieuzugehörigkeit und Mundgesundheit besteht. Lösungsansätze liegen nicht in der vermehrten zahnmedizinischen Betreuung der Kinder, sondern in einer allgemeinen Verbesserung der lokalen Lebens-, Wohn- und Arbeitsbedingungen sowie erhöhten Bildungs- und damit Lebenschancen der sozial benachteiligten Schichten.
Abstract
Our objective was to study why inequalities in dental health persist in the German health care system. The introduction of group prophylaxis in the German social law book in 1988 had an important influence on oral health in general but could not prevent an unequal distribution of poor oral health. It is well known that “health” and “disease” are influenced by diverse factors and are subject to many psychosocial influences mainly independent of the health care system. In this article we analysed relevant studies and their results from the international literature of medical psychology and sociology as well as dentistry. All studies reported a relationship between social class and oral health. The results show that solutions can be found not in better dental cures and prevention for children but in amelioration of local community life, better working conditions, better housing, higher education, and more chances for the social deprived classes.
