Zusammenfassung
Hintergrund
Eine unzureichende Jodversorgung in der Schwangerschaft hat nicht nur allgemeine negative Folgen für die Entwicklung des Feten, sondern auch besonders für die spätere neurophysiologische Leistung des Kindes. Durch jahrelange Aufklärungstätigkeit, u. a. durch den Arbeitskreis Jodmangel, hat sich die Jodversorgung der Bevölkerung deutlich verbessert bzw. stabilisiert. Es muss allerdings davon ausgegangen werden, dass gewisse Bevölkerungsgruppen, besonders solche, die einen erhöhten Jodbedarf wie z. B. Schwangere und Stillende aufweisen, davon nicht ausreichend profitieren.
Ergebnisse
Die Ursache des Jodmangels bei Schwangeren und Stillenden dürfte u. a. darin zu suchen sein, dass Ärzte zur Strumaprophylaxe in der Schwangerschaft keine Jodtabletten mehr verordnen können und deshalb auch eine diesbezügliche Beratung meist ausbleibt. Sozial Schwache können sich häufig keine Jodsupplemente kaufen; darüber hinaus sehen sie oft die Notwendigkeit nicht. Dies schlägt sich in der Häufigkeitszunahme an diagnostizierten Hypothyreosen bei Schwangeren nieder.
Schlussfolgerung
Es wäre wünschenswert, wenn Ärzte bereits vor einer (geplanten) Schwangerschaft (spätestens nach Eintritt einer Schwangerschaft) durch geänderte Mutterschaftsrichtlinien in der Lage wären, eine Strumaprophylaxe einzuleiten.
Abstract
Background
An insufficient iodine supply during pregnancy is disastrous for the development of the fetus, in particular for its later neurophysiological capabilities. A long-term public education campaign has resulted in an improved iodine intake which has been stabilized at an acceptable level. Nevertheless, there are groups within the population which still have deficiencies in iodine uptake, for example those with a more than an average requirement such as women during pregnancy and lactation.
Results
One cause of insufficient iodine supply is the fact that obstetricians are no longer allowed to prescribe iodine supplements for pregnant and lactating women, and therefore physicians do not discuss these problems often enough. Frequently women from lower social classes cannot afford to buy supplements. From the year 2003 onwards, this has clearly resulted in an increase of the frequency of hypothyroidism.
Conclusions
Gynaecologists should provide advice to their patients before planning a pregnancy and it should be possible to prescribe supplements. This should be regulated in the guidelines for pregnancy care.
