Zusammenfassung
Die diabetische Retinopathie und das diabetische Makulaödem stellen wesentliche okulare Komplikationen bei Diabetes mellitus dar. Die diabetische Retinopathie nimmt den 3. Platz der Erblindungsursachen in Deutschland ein, und sie ist ein eigenständiger Risikofaktor für kardiovaskuläre Mortalität. Basis der Diagnosestellung und Stadienbestimmung ist die augenärztliche Untersuchung mit ergänzender apparativer Diagnostik. Die klassische Standardtherapie der proliferativen diabetischen Retinopathie ist die panretinale Laserkoagulation. Das diabetische Makulaödem wird mittels intravitrealer operativer Medikamentenapplikation (IVOM) von vorwiegend VEGF-Inhibitoren (VEGF: vaskulärer endothelialer Wachstumsfaktor [„vascular endothelial growth factor“]) behandelt. In neuerer Zeit wurden zusätzliche Medikamente für die IVOM-Therapie zugelassen, welche die Möglichkeit verlängerter Dosierungsintervalle dieser chronischen Therapie versprechen. Als weiterer Aspekt nimmt das augenärztliche Screening eine entscheidende Stellung ein, wobei diesbezüglich in der aktuellen NVL Typ-2-Diabetes (NVL: Nationale VersorgungsLeitlinie) aus dem Jahr 2023 Empfehlungen gegeben werden. Zudem ergeben sich in diesem Bereich Perspektiven bezüglich der Nutzung digitalisierter Anwendungen sowie der künstlichen Intelligenz.
Abstract
Diabetic retinopathy and diabetic macular oedema are significant ocular complications of diabetes mellitus. Diabetic retinopathy is the third common cause of blindness in Germany and is an independent risk factor for cardiovascular mortality. Diagnosis and stage determination are based on ophthalmological examination and additional instrumental diagnostics. Standard classical treatment for proliferative diabetic retinopathy is panretinal photocoagulation. Diabetic macular oedema is treated with intravitreal drug injection of mainly vascular endothelial growth factor (VEGF) inhibitors. More recently, additional medications have been approved for intravitreal injections, which promise the possibility of extended dosing intervals for this chronic therapy. Moreover, ophthalmological screening plays a crucial role, with recommendations being made in this regard in the current German national care guideline for type 2 diabetes (Nationale VersorgungsLeitlinie Typ-2-Diabetes) from 2023. In addition, there are prospects in this area for the use of digitalized applications and the use of artificial intelligence.
