Zusammenfassung

Die Therapie des Hämorrhoidalleidens erfolgt stadien- bzw. gradadaptiert. Der überwiegende Anteil der Patienten kann konservativ-interventionell behandelt werden, jedoch benötigen etwa 16 % eine operative Therapie. Hierbei stehen sowohl mehrere konventionell resezierende als auch pexierende Verfahren zur Verfügung, die differenziert eingesetzt werden sollten. Das Ziel des „tailored approach“ ist, anhand von Symptomatik, Lokalbefund und Erwartungshaltung des Patienten das optimale Verfahren zu wählen. Die verschiedenen Fragenstellungen bei der Indikation zum jeweiligen Eingriff werden näher erläutert.

Abstract

The treatment of hemorrhoids is adapted to the degree or grade of the disease. The majority of patients can be treated with conservative/interventional therapy, but about 16% require surgery. There are several conventional, resecting as well as pexing procedures available, which should be used in a differentiated manner. The aim of the tailored approach is to select the optimal procedure on the basis of symptoms, local findings and the patient’s expectations. The various issues associated with each procedure are discussed in detail.
