Zusammenfassung
Die Prävalenz fortgeschrittener Lebererkrankungen nimmt weltweit zu. Auch bei potenziell kurativem Therapiekonzept, z. B. durch Lebertransplantation bei Leberzirrhose oder einem hepatozellulären Karzinom, ist die 5‑Jahres-Mortalität sehr hoch und die progrediente Verminderung der hepatischen Funktion kann selten aufgehalten werden. Patienten mit fortgeschrittenen Lebererkrankungen leiden daher unter der Zunahme der leberspezifischen Symptomlast und einer eingeschränkten Lebensqualität. Bisher fehlen Konzepte zur Verbesserung der palliativen Grundversorgung hepatologischer Patienten und es gibt nur wenige orientierende Behandlungspfade zur Linderung der leberspezifischen Symptomlast. Die vorliegende Übersicht dient der Sensibilisierung von Primärversorgern und den behandelnden Spezialisten für die Bedürfnisse dieser Patienten sowie wesentliche Bereiche der palliativen Grund- und spezialisierten Palliativversorgung. Insbesondere sollte bei fortgeschrittenen Lebererkrankungen nach aktueller Situation der Patientenwunsch abgefragt, eine ausreichende Schmerzkontrolle mit an die Leberfunktion angepasster Dosierung angestrebt und leberspezifische Symptome – Aszites, hepatische Enzephalopathie, Übelkeit, Juckreiz – behandelt werden. Insgesamt sollte eine frühere und verbesserte Palliativbehandlung von Patienten mit fortgeschrittener Lebererkrankung durch Integration palliativer Aspekte in die Primärversorgung angestrebt werden.
Abstract
The prevalence of advanced liver disease is increasing worldwide. Even with potentially curative therapeutic regimens, such as liver transplantation for cirrhosis or hepatocellular carcinoma, 5‑year mortality is very high and progressive decline in hepatic function can rarely be avoided. Patients with advanced liver disease therefore suffer from an increase in liver-specific symptom burden and a reduced quality of life. To date, concepts to improve palliative primary care for hepatologic patients are lacking, and there are few guiding treatment pathways to alleviate liver-specific symptom burden. The purpose of this review is to raise awareness among primary care providers and treating specialists about the needs of these patients and essential areas of palliative primary and specialized palliative care. Specifically, in advanced liver disease, patient wishes should be queried according to the current situation, adequate pain control should be sought with dosing adjusted to liver function, and liver-specific symptoms—ascites, hepatic encephalopathy, nausea, pruritus—should be addressed. Overall, earlier and improved palliative care of patients with advanced liver disease should be sought by integrating palliative aspects into primary care.
