Zusammenfassung
Körperliches Training ist eine der zentralen Therapiestrategien bei Adipositas, metabolischem Syndrom und Diabetes mellitus Typ 2. Dieses wird auch von den behandelnden Hausärzten und Diabetologen umfassend empfohlen. Jedoch ist die praktische Umsetzung für den Patienten häufig ineffizient und erfolglos. Dieses liegt sowohl an den Patienten wie auch an den Therapeuten. So fehlen eingängige Strategien zu Lebensstilumstellung, die auf der einen Seite allgemein angewendet werden können, zum anderen aber auch individuell auf den Patienten abgestimmt werden. Dieses Konzept verfolgt grundsätzliche Prinzipien, die u. a. ein individueller Trainingsplan beinhaltet, der eindeutige Angaben zur Art der körperlichen Aktivität (Ausdauer- oder Krafttraining), Dosis (Häufigkeit und Belastungsintensität) und zum Aufbau des Trainings gibt. Dieser Inhalt ist mit einer Motivationsstrategie gekoppelt, die darauf abzielt, den Patienten in das aktive Konzept zu integrieren und weniger Empfehlungen zu geben als diese mit bestimmten Kommunikationsstrategien für den Patienten selber offensichtlich zu machen.
Abstract
Physical activity is a key therapeutic component in patients with obesity, metabolic syndrome and type 2 diabetes. However, overall compliance of patients to increase physical activity is low, which is mainly dependent on a lack in communication skills of the treating physician regarding lifestyle implementation and long-term motivation of the patient. Therefore, physicians should be trained regarding exercise protocols for obese and diabetic patients including prescription of tailored exercise intervention including mode of exercise, intensity, duration and frequency. This should be combined with simple nutritional advices and regular consultation, a strategy that will eventually improve short- and long-term compliance to lifestyle intervention in these patients.
