Zusammenfassung
Während Clindamycintherapie wegen Sinusitis maxillaris klagte eine 31-jährige Patientin plötzlich über Dysphagie. Die Ösophagoskopie zeigte eine 7 cm lange zirkumferenzielle Plattenepitheldesquamation. Histologisch ergab sich eine fibrinopurulente Nekrose, passend zu einer medikamentös induzierten Ösophagusläsion. Diese Läsionen sind für mehr als 100 Arzneimittel beschrieben, ihre Pathogenese ist aber noch nicht geklärt. Antibiotika stellen in mehr als 50% der Fälle das auslösende Agens dar. – Unter Einnahme von PPI, Steroiden und Sucralfat sowie nach Absetzen von Clindamycin war die Patientin rasch beschwerdefrei.
Abstract
During clindamycin treatment for maxillary sinusitis, a 31-year-old woman suddenly complained of dysphagia. Endoscopy showed a 7 cm long circular ulceration of the lower esophagus and histological findings of ulceration confirmed a drug-induced esophageal injury. Such injuries have been described for more than 100 different drugs but the exact pathogenesis is still unclear. Antibiotics are the origin in more than 50% of the cases. Under treatment with proton pump inhibitors (PPI), steroids, antacids and withdrawal of clindamycin, an early and complete symptomatic recovery was achieved.
