Zusammenfassung
Entscheidendes Merkmal der Ernährungsmedizin in der Palliativmedizin ist der Wechsel des Therapieziels. Nicht mehr die Wiederherstellung physiologischer Verhältnisse durch Deckung der Bedarfswerte von Nährstoffen und Flüssigkeit, sondern die Kontrolle von Symptomen steht nun an erster Stelle. Je nach Krankheitsphase kann lediglich das Stillen von Hunger und Durst angebracht sein – dies gehört zur obligaten Basisbetreuung in der Sterbebegleitung – oder aber eine bedarfsdeckende Ernährung zur bestmöglichen Linderung von Schwäche sowie Erhalt von Leistungsfähigkeit und Lebensqualität. Dies sollte in abgestuftem Vorgehen vorzugsweise mittels oraler Ernährung erfolgen und kann, wenn die orale Ernährung nicht mehr in ausreichendem Maße möglich ist, als enterale Sondenernährung oder häusliche parenterale Ernährung vorgenommen werden. Bei fortgeschrittener Demenzkrankheit kündigen Essensprobleme in der Regel die terminale Krankheitsphase an und stellen keine Indikation für eine Sondenernährung mittels PEG dar.
Abstract
A key feature of nutritional medicine in palliative care is the change of therapeutic goals. The restitution of physiological conditions by the provision of nutrients and fluid according to standard requirements is no longer a primary goal. Symptom control has now taken its place. According to the stage of disease progression either just to satisfy hunger and thirst may be appropriate as these measures are part of the obligatory basic care of the dying patient or nutrition meeting full nutrient and fluid requirements may be warranted in order to improve fatigue, impaired physical function and loss of quality of life. Nutritional therapy should be offered in a step-wise fashion with normal oral nutrition as the first line. If this is impossible or impractical enteral or home parenteral nutrition should be considered. In patients with advanced dementia, eating problems are a typical indicator of the terminal disease phase when enteral tube feeding by percutaneous endoscopic gastrostomy (PEG) is not indicated.
