Zusammenfassung
Die Stuhlinkontinenz ist ein häufiges Problem, insbesondere des alternden Menschen, mit vielfältigen negativen sozialen und medizinischen Auswirkungen. Durch geeignete diagnostische Verfahren lässt sich bei vielen Betroffenen die Ursache der Stuhlinkontinenz aufdecken. Die Basisdiagnostik umfasst die Anamnese, die digitale Untersuchung und die Proktoskopie. Zur Spezialdiagnostik zählen die anorektale Manometrie, die Endosonographie und das EMG des M. externus. Durch gezielte Therapiemaßnahmen und -algorithmen kann vielfach eine Besserung der Beschwerden bis hin zur kompletten Kontinenz erreicht werden. Die konservativen Maßnahmen beinhalten hygienische Maßnahmen, medikamentöse Ansätze sowie übende Therapien, z. B. Beckenbodengymnastik, Biofeedback und Elektrostimulation. Die chirurgischen Optionen reichen von Repair-Operationen (z. B. direkte Defektnaht) über die Implantation eines sakralen Schrittmachers bis hin zu Ersatzoperationen wie der dynamischen Gracilisplastik und dem Einbau eines Neosphinkters. Die Differenzialindikation der konservativen und operativen Therapieverfahren ist abhängig von der pathophysiologisch gestörten Funktion und der Therapiefähigkeit des Patienten im fortgeschrittenen und hohen Lebensalter.
Abstract
Fecal incontinence is a common problem, particularly for the elderly, that can have a number of negative social and medical consequences. Correct diagnosis will show its cause in many cases. Basic diagnosis calls for anamnesis, digital examination, and proctoscopy. Special diagnostic tools include anorectal manometry, endosonography, and EMG of the external anal sphincter muscle. Using correct algorithms, the degree of incontinence can be reduced or the continence even restored. Conservative treatment involves hygiene, medical therapy, and personal training such as pelvic floor exercises, biofeedback, and electrostimulation. The surgical options range from repair operations such as sphincteroplasty to sacral nerve stimulation, dynamic graciloplasty, and implantation of an artificial sphincter. Which treatment to select — conservative or radical — depends on the pathophysiology of the disturbance and the patient’s cognitive state and possible comorbidities.
