Zusammenfassung
Die Adipositasprävalenz ist in den letzten Jahrzehnten in den Industriestaaten auf etwa 20% angestiegen. Mit einem weiteren Anstieg auch der adipositasassoziierten Komorbiditäten ist in den nächsten Jahren zu rechnen. Adipositas ist ein wichtiger Risikofaktor für die nichtalkoholische Fettlebererkrankung, die Refluxkrankheit, das Gallensteinleiden und für Tumorerkrankungen. Menschen mit einer Adipositas haben ein deutlich erhöhtes Risiko für die Entwicklung von Kolonkarzinom, Gallenblasenkarzinom, Adenokarzinom des Ösophagus und das hepatozelluläre Karzinom. Das viszerale Fettgewebe, gemessen als Taillenumfang oder WHR, korreliert dabei besser mit dem Komorbiditätsrisiko als der BMI. Neben der Insulinresistenz und oxidativem Stress spielt die Sekretion von Adipokinen (z. B. Leptin) und proinflammatorischen Zytokinen des viszeralen Fettgewebes eine wichtige pathogenetische Rolle. Diese bei weitem nicht aufgeklärten Pathomechanismen stellen das Bindeglied der viszeralen Adipositas zu vielen Organsystemen und zur Tumorgenese dar. Zur Bestimmung des individuellen Risikoprofils und für die Therapie unter präventiven Gesichtspunkten gewinnt die Adipositas immer mehr an Bedeutung.
Abstract
In the industrial countries the prevalence of adipositas has risen to approximately 20% during the last decades. In addition, a further rise in adipositas-related comorbidities is to be expected. Adipositas is an important risk factor for nonalcoholic fatty liver diseases, reflux, cholelithiasis, and tumour diseases. Adipose patients have a significantly higher risk of developing colon carcinoma, gallbladder carcinoma, oesophageal adenocarcinoma, and hepatocellular carcinoma. Visceral fat measured as waist circumference or waist:hip ratio correlates more with the comorbidity risk than body mass index. Apart from insulin resistance and oxidative stress, secretion of adipokins such as leptin and proinflammatory cytokines of visceral fatty tissue play an important pathogenic role. Still not completely clarified, these pathomechanisms represent the link between visceral adipositas and many organ systems and tumourigenesis. Regarding preventive aspects, adipositas is increasingly relevant concerning the determination of risk profiles and therapy.
