Summary
In the last 20 years, accompanying psychotherapeutic treatment in patients with serious physical illness has become a component of the activities of psychosomatic consultation and liaison services in hospital. Conceptualization of this activity on a psychodynamic basis has been described. The goals of psychotherapeutic interventions are stabilization of the patient’s defences and reinstatement of structured coping with the disease as well as with caregivers and significant others. Effect factors are an empathic relationship, verbalization of stressful negative feelings, focussing on current problems and resource orientation. The main elements of therapy are a narrative discussion structure, psychodynamic understanding of the current illness situation in relation to the patient’s life history, active listening, focus formulation and systemic way of viewing the situation. Problems arise because of negative countertransfer on the psychotherapist's part and pronounced denial processes on the patient’s part. From the psychotherapist, implementation requires continuous presence and regular reports and consultations with the team of caregivers.
Zusammenfassung
In den letzten 20 Jahren ist die begleitende psychotherapeutische Behandlung bei körperlich Schwerkranken fester Bestandteil der Tätigkeit von psychosomatischen Konsiliar- und Liaisondiensten im Krankenhaus geworden. Eine Konzeptualisierung dieser Tätigkeit auf psychodynamischer Basis wird beschrieben. Ziele der psychotherapeutischen Interventionen sind die Stabilisierung der Abwehr des Patienten und die Rückgewinnung eines strukturierten Umgangs mit der Krankheit mit den Behandlern und nahen Bezugspersonen. Wichtige Wirkfaktoren sind eine empathische Beziehung, Verbalisierung von belastenden negativen Gefühlen, Fokusieren auf aktuelle Probleme und Ressourcenorientierung. Hauptsächliche Therapieelemente sind eine narrative Gesprächsstruktur, ein psychodynamisches Verständnis der aktuellen Krankheitssituation auf dem Hintergrund der Lebensgeschichte, aktives Zuhören, Fokusformulierung und systemische Sichtweise. Probleme entstehen durch negative Gegenübertragung des Psychotherapeuten und ausgeprägte Verleugnungsvorgänge auf Patientenseite. Die Umsetzung erfordert vom Psychotherapeuten kontinuierliche Präsenz und regelmäßige Rücksprachen und Konsultationen mit dem behandelnden Team.
