Der Einfluss von langwirksamen intramuskulär verabreichten Antipsychotika auf intramurale Zwischenfälle bei Maßnahmepatienten mit Schizophrenie

The effective psychopharmacological treatment is an important issue for a good legal prognosis of violent offenders suffering from schizophrenia. Due to poor compliance long-term depot antipsychotics have a special position in the risk management of these patients. There are no recent studies on the influence of different long-term depot antipsychotics on acts of institutional violence. A group of offenders suffering from schizophrenia with long-acting haloperidol was compared with a second group with long-acting risperidone. Given a similar psychopathological level, the anti-aggressive effect of both antipsychotics was comparable.

Zusammenfassung:

Die effektive psychopharmakologische Behandlung ist ein Schlüssel für eine günstige Kriminalprognose von gewalttätigen Maßnahmepatienten mit Schizophrenie. Dabei haben aufgrund der für gewöhnlich schlechten medikamentösen Compliance antipsychotisch wirksame Depotpräparate einen herausragenden Stellenwert im Risikomanagement dieser Patientengruppe. Der Einfluss der unterschiedlichen Depotpräparate auf Zwischenfälle im Maßnahmevollzug, ein wichtiger Indikator für die Legalprognose nach der bedingten Entlassung, wurde bisher nicht ausreichend untersucht.

In einer retrospektiven Studie wurden zwei Samples verglichen, die auf langwirksames intramuskuläres Risperidon bzw. auf Haloperidol eingestellt waren. Bei gleicher psychopathologischer Ausgangslage erwiesen sich beide Präparate als gleichwertig in ihrer antiaggressiven Wirksamkeit.
