Zusammenfassung
Patienten, die an psychiatrischen Störungen leiden, können aufgrund der damit assoziierten Symptomatik mitunter auch gewalttätiges Verhalten zeigen. Konsequenterweise ergibt sich daraus für Mitarbeiter psychiatrischer Einrichtungen nicht nur die Notwendigkeit, sondern auch die Verpflichtung, selbst- oder fremdgefährdendes Verhalten auch unter Anwendung von Zwangsmaßnahmen und Eingriffen in die persönliche Freiheit zu verhindern. Das Management von Notfallsituationen kann in vier Schwerpunkte gegliedert werden: Das frühzeitige Erkennen von Risikofaktoren für aggressives Verhalten, die Prävention von Aggressionen, das Vorgehen bei akutem auto- und/oder fremdaggressiven Verhalten und die Nachsorge. Um in Notfallsituationen einen strukturierten und korrekten Umgang mit Fixierungen, Isolierungen und Notfallmedikation zu ermöglichen, sind regelmäßige Aus- und Fortbildungsveranstaltungen erforderlich.
Summary
Patients suffering from psychiatric disorders may show violent behavior caused by disorder specific symptoms. Consequently, mental health staff are under an obligation to prevent aggressive behavior directed against others or the patients themselves even by employing force and restricting the personal freedom of patients. For the management of such emergency situations four factors need to be considered: the assessment of risk factors for aggressive behavior, the prevention of aggression, management of aggressive behavior and supervision of the team. Continuing education on a regular basis is a prerequisite to ensure a structured and correct management of emergency situations.
