Zusammenfassung
Zunehmend stärker ist ein klinisches Engagement des Konsiliarpsychiaters in speziellen, erst durch den Fortschritt der modernen Medizin selbst geschaffenen klinischen Settings wie Intensivstationen, Transplantationseinheiten und herzchirurgischen Stationen gefordert. In diesem Kontext ist das Risiko einer posttraumatischen Belastungsstörung (PTSD) zu nennen. Ergebnisse aus drei eigenen empirischen Untersuchungen zur psychiatrischen Komorbidität und gesundheitsbezogenen Lebensqualität bei Patienten nach akutem Lungenversagen (ARDS) im Langzeitverlauf, nach orthotoper Lebertransplantation (LTx) im Mittelzeitverlauf und nach Herzoperation unter Einsatz der Herz-Lungen-Maschine im Kurzzeitverlauf werden dargestellt. In der Zusammenschau kann festgehalten werden, dass PTSD als Folge schwerer körperlicher Erkrankungen und notwendiger Therapiemassnahmen nicht selten auftritt. Bleibt sie unbehandelt, kann PTSD, speziell das Auftreten von intrusiven Rekollektionen, Vermeidungsverhalten und Symptomen der autonomen Hyperaktivität, das Leben der betroffenen Patienten im Verlauf zum Teil stärker beeinträchtigen als die zugrunde liegende somatische Krankheit. Dies gilt auch für subsyndromale Verlaufsformen. Um gefährdete Patienten zeitgerecht zu diagnostizieren und einer adäquaten Therapie zuzuführen, ist eine enge Zusammenarbeit zwischen somatisch und konsiliarpsychiatrisch tätigen Ärzten essenziell.
Summary
Developing strategies for detecting and treating intensive care-, transplantation surgery-, and heart surgery-related mental disorders represents an increasingly important challenge to consultation – liaison (C – L) psychiatry, as there is evidence stemming from newly presented studies that the remarkable advances in intensive care treatment, organ transplantation medicine and cardiac surgery with cardiopulmonary bypass within the past decade have an important impact on the general hospital inpatients' psychosocial and psychological outcome. In this context, the risk of developing posttraumatic stress disorder (PTSD) has to be considered. Three own empirical studies examining psychiatric and psychosocial outcome of long – term acute respiratory distress syndrome (ARDS) survivors, intermediate – term survivors of orthotopic liver transplantation (OLT), and short–term patients of cardiac surgery employing cardiopulmonary bypass are presented. In conclusion, PTSD symptoms following severe medical conditions and treatment are not rare. If they are untreated, PTSD symptoms such as intrusive recollections, avoidance and hyperarousal may impair the patients' quality of life more than the primary disease. This seems to be also true for a subthreshold PTSD. To adequately diagnose and treat patients at risk of developing PTSD, close collaboration between physicians of all subspecialties and C – L psychiatrists will be necessary.
