Zusammenfassung
Hintergrund
Nichttuberkulöse Mykobakteriosen der Lunge (NTM-LD) sind chronisch verlaufende Infektionen, die gehäuft in Zusammenhang mit prädisponierenden Lungenerkrankungen (z. B. COPD [chronisch obstruktive Lungenerkrankung], Bronchiektasen) auftreten und im klinischen Alltag Ärzte und betroffene Patienten oft vor Herausforderungen stellen.
Ziel der Arbeit
Es erfolgt eine überblickhafte Darstellung der Epidemiologie, Diagnostik und Therapie von NTM-LD.
Material und Methoden
Anhand aktueller wissenschaftlicher Erkenntnisse und der aktuellen Leitlinienempfehlungen wird die Komplexität der Behandlung von NTM-LD dargelegt und anhand von 2 auslösenden Spezies (Mycobacterium [M.] avium-Komplex und M. abscessus) veranschaulicht.
Ergebnisse
NTM-LD können in variabler Ausprägung vorliegen. Die Diagnose richtet sich nach etablierten klinischen, bildgebenden und mikrobiologischen Kriterien. Die Entscheidung über eine Therapie richtet sich nach weiteren Kofaktoren wie Begleiterkrankungen, nachgewiesener NTM-Spezies, Resistenzlage, Schwere der Erkrankung und zu erwartendem Therapie-Outcome. In manchen Fällen kann ein abwartendes, kontrollierendes Prozedere („watchful waiting“) indiziert sein.
Diskussion
Pulmonale NTM-Infektionen erfordern eine langfristige (≥ 12 Monate), teils parenteral applizierte und mit signifikanter Toxizität verbundene Therapie. Zur optimalen Diagnostik und Abwägung für und gegen eine Therapie sollte eine frühzeitige Vorstellung betroffener Patienten an einem Fach‑/Kompetenzzentrum erfolgen.
Abstract
Background
Nontuberculous mycobacterial infections of the lung (NTM-LD) are diseases with a chronic course, that often occur in conjunction with predisposing lung diseases (e.g., chronic obstructive lung disease. COPD, bronchiectasis) and frequently present challenges to physicians and affected patients in clinical practice.
Objective
An overview of the epidemiology, diagnostics, and treatment of NTM-LD is presented.
Material and methods
The complexity of the treatment of NTM-LD is outlined based on the current scientific evidence and current guideline recommendations and is illustrated using two exemplary species (Mycobacterium avium complex and M. abscessus).
Results
NTM-LD can occur in variable grades of severity. The diagnosis is based on established clinical, imaging and microbiological criteria. The decision on treatment depends on other co-factors, such as concomitant diseases, detected NTM species, antimicrobial resistance, severity of the disease and expected treatment outcome. In some cases, an approach of watchful waiting can be indicated.
Conclusion
Pulmonary NTM infections require long-term (≥ 12 months), in some cases parenterally administered treatment that is associated with a significant toxicity. For optimal diagnostics and weighing up for or against a form of treatment, early referral of proven or suspected patients to a competence center with experience in treating NTM infections should be carried out.
