Zusammenfassung
Bezüglich Transition bei Lungenerkrankungen gibt es 2 große Themen: 1) Kenntnis über den frühkindlichen Einfluss auf Erkrankungen mit klinischer Manifestation im Erwachsenenalter und 2) Übergang von pädiatrischer zu internistisch-pneumologischer Versorgung bei definierten chronischen Erkrankungen. Mittlerweile ist es allgemein akzeptiert, dass frühkindliche und sogar vorgeburtliche Faktoren eine wesentliche Rolle für die Lungengesundheit im Erwachsenenalter spielen. Das Konzept des „Tracking“ bedeutet dabei, dass eine frühkindliche Einschränkung der Lungenfunktion dazu führt, dass die maximal erreichte Lungenfunktion zu Beginn des Erwachsenenalters geringer ausfällt und damit das Risiko für eine frühe Manifestation einer COPD (chronisch obstruktive Lungenerkrankung) steigt. Insofern ist im Kontext mit der Evaluation erwachsener Patienten die Kenntnis der pädiatrischen Anamnese auch für den internistischen Pneumologen wichtig. Transition der medizinischen Versorgung könnte bei entsprechenden Erkrankungen bedeuten: präventive Begleitung von Jugendlichen und jungen Erwachsenen, bei denen entsprechende Risiken aus der frühen Kindheit bekannt sind. Ein strukturierter Transitionsprozess ist wünschenswert, um die Jugendlichen auf den Wechsel der Versorgung vorzubereiten, den Informationsaustausch zwischen den Disziplinen sicherzustellen und im Sinne der Patienten eine erhöhte Morbidität durch Destabilisierung während eines unstrukturierten Wechsels zu verhindern. Nachfolgend werden zum einen die frühkindlichen Faktoren und das Modell des „Tracking“ erläutert, aber auch klinische und psychosoziale Aspekte der Transition an den Beispielen „bronchopulmonale Dysplasie“ und „schweres Asthma“ bei Jugendlichen und jungen Erwachsenen dargestellt, um schließlich aktuelle Leitlinien und praktische Aspekte zu diskutieren.
Abstract
The transition from pediatric to adult care of pulmonary diseases includes two big topics: 1) knowledge of factors influencing lung development during early childhood with clinical manifestation in adulthood and 2) transition from pediatric to adult pneumological care of patients with defined chronic lung diseases. It is now generally accepted that early childhood and even prenatal factors play a substantial role for lung health in adulthood. With respect to the concept of tracking, an early childhood impairment of lung function means that the maximum lung function achieved in young adulthood is lower and therefore the risk of an early manifestation of chronic obstructive pulmonary diseases (COPD) increases. Thus, in the context of the evaluation of adult patients, knowledge of the pediatric history is also important for the pneumologist Transition of the medical care could mean for appropriate diseases: preventive accompaniment of adolescents and young adults for whom corresponding risks are known from early childhood. A structured transition process would be desirable to prepare the adolescents for the change in care, to ensure information exchange between the disciplines and for the benefit of the patient to avoid an increased morbidity by destabilization during an unstructured transition. This article elucidates the early childhood factors and the model of tracking and presents the clinical and psychosocial aspects of transition using the examples of bronchopulmonary dysplasia and severe asthma in adolescents and young adults in order to finally discuss the current guidelines and practical aspects.
