Zusammenfassung
In Deutschland und Europa zeigt sich eine steigende Prävalenz von beatmungspflichtigen Patienten. Dies geht mit steigenden Kosten für das Gesundheitssystem einher. Hier können additive telemedizinische Ansätze nicht nur zu einer Senkung von Gesundheitsausgaben, sondern auch zu einer verbesserten, außerklinischen Versorgung der Patienten beitragen. So können bereits heutzutage Beatmungsgeräte pneumotachographisch abgeleitete Therapiedaten (z. B. über Nutzungsdauer, Leckage, Atemfrequenz, Atemzugsvolumina) telemedizinisch übertragen. Bevor diese Daten jedoch in medizinische Entscheidungen einfließen können, muss evidenzbasiert untersucht werden, inwiefern die Daten zuverlässig sind und ob durch ihren Einsatz das Outcome für Patienten wirklich verbessert werden kann. Gleichzeitig müssen hierbei gesundheitsökonomische Aspekte betrachtet werden. Der Artikel beschreibt technologische Fortschritte im Bereich der Telemedizin in der außerklinischen Beatmung und diskutiert auf Basis aktueller Publikationen die bestehende Evidenz in diesem Bereich. Gleichzeitig wird unter Beachtung medizinrechtlicher Aspekte das Potenzial additiver telemedizinischer Ansätze in integrierten Versorgungskonzepten für außerklinisch beatmete Patienten diskutiert.
Abstract
In Germany as well as in Europe an increasing prevalence of patients requiring out-of-hospital mechanical ventilation can be observed. This is accompanied by increasing costs for healthcare systems. Additive telemedical approaches can contribute not only to a reduction of healthcare costs but also to an improved out-of-hospital care of patients. Nowadays, ventilators can already remotely transmit pneumotachographically derived treatment data (e.g. on duration of use, leakage, respiratory rate and breathing volumes); however, before these data can be incorporated into medical decisions, there is a need for evidence-based research on to what extent telemedically transmitted data are reliable and whether the use can really improve patient outcomes. In addition, health economic aspects must be considered. This article describes the technological advances in telemedicine for out-of-hospital ventilation and discusses the existing evidence in this field based on recent publications. Beyond that, the potential of additive telemedical approaches in integrated care concepts for out-of-hospital ventilation of patients is discussed, also taking medicolegal aspects into account.
