Zusammenfassung
Interferon-γ Release Assays (IGRAs) sind In-vitro-Immunverfahren, die vor einigen Jahren als Alternative zum Tuberkulinhauttest (THT) zur Diagnose der latenten Tuberkuloseinfektion (LTBI) eingeführt wurden. Aufgrund ihrer hohen Spezifität insbesondere bei Personen mit Kreuzreaktionen, wie z.B. der BCG-Impfung, können IGRAs die Zahl falsch-positiv Getesteter drastisch reduzieren. Obwohl ihre gepoolte Sensitivität unter 90% liegt, können IGRAs auch für die Diagnose der aktiven Tuberkulose (TB) in Kombination mit anderen Maßnahmen hilfreich sein. Vor der Gabe von TNF-α-Inhibitoren ist für das Tuberkulose-Screening primär nur noch der Einsatz von IGRAs zu empfehlen.
Abstract
Interferon-γ release assays (IGRAs) are in vitro immunoassays that have been introduced in recent years as an alternative to the tuberculin skin test (TST) for diagnosis of latent tuberculosis (LTBI). Due to their high specificity, especially in subjects with confounding factors such as BCG vaccination, IGRAS may rigorously reduce the number of persons with false-positive test results. Although pooled sensitivity is below 90%, IGRAs may be helpful for diagnosing active tuberculosis (TB) when combined with other tools. To detect LTBI before initiating TNF-α inhibitor therapy primarily only IGRAs are recommended.
