Zusammenfassung
Asbestfaserstäube sind in der Lage, fibrogene Wirkungen des Lungenparenchyms im Sinne einer Asbestose, meist in Form von hyalinen und verkalkten Pleuraplaques hervorzurufen. Weitere benigne Reaktionen sind Pleuraergüsse (Asbestpleuritis), diffuse Pleurafibrosen und Kugelatelektasen. Plaques sind die häufigste pleurale Manifestation und gelten als Marker der stattgehabten Asbestexposition. Hinweise auf die Asbestgenese bei Lungenparenchymerkrankung sind eine hohe Intensität der Exposition und die Kombination der Lungenfibrose mit Pleuraplaques.
Die nachgewiesene kanzerogene Potenz von Asbestfaserstäuben stellt ein lebensbedrohliches Faktum dar. Asbestassoziierte Tumoren sind das Lungenkarzinom, Larynxkarzinom und das Mesotheliom.
Bei benignen Pleura- und Lungenerkrankungen orientiert sich die Bemessung der Minderung der Erwerbsfähigkeit an den Lungenfunktionsdaten und am Röntgenbefund. Zur Anerkennung von Lungenkrebs als Berufserkrankung muss eine Asbestose, eine asbestassoziierte Erkrankung der Pleura oder eine kumulative Asbestfaserfeinstaubdosis am Arbeitsplatz von mindestens 25 Faserjahren vorliegen. Alternativ wird die histologische Diagnose einer Minimalasbestose gefordert.
Abstract
Asbestos particles are able to provoke fibrinogenic effects of the lung parenchyma in the sense of an asbestosis, mostly in the form of hyalines and calcified pleural plaques. Further benign reactions are pleural effusion (asbestos pleurisy), diffuse pleural fibrosis and rounded atelectasis. Plaques are the commonest pleural manifestation and are considered to be markers of exposure to asbestos. Indications of an asbestos genesis of lung parenchymal disease are a high intensity of exposure and the combination of pulmonary fibrosis and pleural plaques.
The proven carcinogenic potency of asbestos particles represents a life-threatening factor. Asbestos-associated tumors are lung cancer, laryngeal carcinoma and mesothelioma.
For benign pleural and pulmonary diseases estimation of the reduction in the capacity to work is oriented towards the data on pulmonary function and the results of X-ray examinations. For the recognition of lung cancer as an occupational disease, an asbestosis, an asbestos-associated pleural disease or a cumulative asbestos fibre fine particle dose at the workplace of at least 25 fibre years must be present. Alternatively, the histologic diagnosis of a minimal asbestosis is necessary.
