Bronchialobstruktion bei 62-jährigem Nichtraucher

Zusammenfassung

Ein 62-jähriger Nichtraucher stellte sich mit Belastungsdyspnoe und Giemen vor; anamnestisch waren wiederkehrende Sinusitiden bekannt. Die Diagnose ergab die Symptomtrias des Kartagener-Syndroms: fixierte, bronchiale Obstruktion, Situs inversus, Bronchiektasie. Bronchialsekret und Abstrich der Nasennebenhöhlen enthielen 2 multiresistente Stämme von Pseudomonas aeruginosa. Die Zilien der Bronchialschleimhaut zeigten elektronenmikroskopisch Defekte der Dyneinarme. Mit i.v.-Antibiotikagabe, Inhalationen und Physiotherapie besserten sich Belastbarkeit und Lungenfunktion.

Abstract

A 62-year-old nonsmoking patient presented with exertional dyspnoe, wheezing, and a history of recurrent sinusitis. Diagnostics showed the typical signs of Kartagener syndrome: fixed airway obstruction, situs inversus, and bronchiectasis. Two strains of multi-drug-resistant Pseudomonas aeruginosa were isolated from the upper and lower airways. Electron microscopy revealed a lack of dynein arms of the cilia of the lower respiratory tract. With an intravenous antibiotic regimen, inhalation therapy, and chest physiotherapy, the patient’s symptoms and lung function parameters improved.
