Zusammenfassung

Die allergische bronchopulmonale Aspergillose (ABPA) stellt eine Komplikation sowohl bei Patienten mit Asthma bronchiale wie auch bei zystischer Fibrose dar. Die Dunkelziffer ist vermutlich hoch, die Diagnosestellung erfolgt häufig mit beträchtlicher Latenz. Die diagnostischen Kriterien umfassen anamnestische Daten, pulmonale Infiltrate, eine Sensibilisierung gegenüber Aspergillus spp. sowie zentrale Bronchiektasen. Ein hoher Gesamt-IgE-Spiegel sowie IgE- und IgG-Antikörper gegen Aspergillus fumigatus sind wegweisend. Die Therapie der Wahl sind Kortikosteroide. Eine zusätzliche antimykotische Therapie sollte bei ausgeprägten Fällen diskutiert werden.

Abstract

Allergic bronchopulmonary aspergillosis (ABPA) is a known complication in patients with bronchial asthma and cystic fibrosis. The estimated number of unknown cases is very high. There is a great latency between symptoms and diagnosis of this disease. Diagnostic criteria involve anamnesis, pulmonary infiltrates, sensitization against Aspergillus species, and predominant central bronchiectasis. IgE and IgG antibodies against Aspergillus fumigatus are important elements for diagnosis. Treatment with corticosteroids is recommended. In severe courses, additional antimycotic therapy should be considered.
