Zusammenfassung
Die chronische pulmonale Hypertonie (PH) ist mit einer verminderten körperlichen Belastbarkeit, eingeschränkten Lebensqualität und schlechten Prognose assoziiert. Eine prospektive, randomisierte Studie hat jetzt erstmals den Effekt von körperlichem Training und Atemtherapie auf Patienten mit schwerer PH untersucht. Die Patienten der Trainingsgruppe wiesen nach 15 Wochen eine signifikante Verbesserung der 6-min-Gehstrecke um im Mittel 111 m (p<0,001) auf. Lebensqualität, NYHA-Klasse, maximale Sauerstoffaufnahme und maximale Belastungsstufe auf dem Fahrradergometer konnten ebenfalls durch die Atem- und Bewegungstherapie signifikant verbessert werden. Diese Studie konnte zeigen, dass Atem- und Bewegungstraining unter stationärer Begleitung sicher und effektiv ist und die medikamentöse Therapie effektiv ergänzen kann.
Abstract
Pulmonary hypertension (PH) is associated with limited quality of life, reduced physical capacity, and poor prognosis due to right heart failure. A recent prospective, randomized study evaluated the effects of exercise and respiratory training in patients with symptomatic PH. At week 15, patients in the training group had improved 6MWD (mean difference 111 m; p<0.001). Exercise training was well tolerated and significantly improved scores of quality of life, WHO functional class, peak oxygen consumption, and achieved workload. This study indicates that respiratory and physical training could be a promising adjunct to medical treatment in severe PH.
