Zusammenfassung

Nokardien sind ubiquitär im Boden verbreitete Mikroorganismen. Infektionen treten gehäuft, aber nicht ausschließlich bei Patienten mit Immundefiziten und mit chronischen Lungenerkrankungen auf. Klinisch sind pulmonal begrenzte und disseminierte Verläufe zu unterscheiden, Letztere haben eine ungünstigere Prognose. Daneben treten isolierte Haut-/Weichteilinfektionen auf. Die Diagnose erfolgt bei klinischem Verdacht mikroskopisch und kulturell aus respiratorischem Material und sterilen Materialien (Blut, Abszesspunktat). Zur Therapie werden vornehmlich Co-trimoxazol (limitierte Formen) und Imipenem/Amikacin (disseminierte Formen) eingesetzt.

Abstract

Nocardia spp. are ubiquitously distributed gram-positive bacteria that cause human infections predominantly in the immunocompromised host and in patients with chronic lung diseases. The clinical spectrum includes localized cutaneous and pulmonary infections and generalized disease which has a serious prognosis. The diagnosis is made by microscopic and cultural detection from respiratory and sterile samples. Therapeutic options include co-trimoxazole and imipenem/amikacin.
