Zusammenfassung
Pleuraergüsse unsicherer oder unklarer Ätiologie stellen ein häufiges klinisches Problem dar. Die diagnostische Punktion und laborchemische Analysen ermöglichen die Unterscheidung zwischen Transsudat und Exsudat. Diese Unterteilung erlaubt die ersten Weichenstellungen hinsichtlich der weiteren diagnostischen und therapeutischen Maßnahmen. Während Transsudate keine weitere Pleuradiagnostik erforderlich machen, sondern die weiteren Schritte in Richtung Diagnostik und Therapie der Grunderkrankung lenken, eröffnet sich beim Exsudat eine breite Differenzialdiagnose, die v. a. entzündliche und maligne Erkrankungen, aber auch andere und seltene Erkrankungen beinhaltet. Das Arsenal diagnostischer Möglichkeiten im Pleuraraum umfasst die Zytologie, die Bakteriologie, ggf. die Pleurablindbiopsie, die konventionelle starre und die innovative semiflexible Pleuroskopie. In dieser Übersicht wird u. a. der aktuelle Stand des evidenzbasierten Vorgehens beim parapneumonischen Erguss und beim Pleuraempyem sowie beim malignen Erguss aus internistisch-pneumologischer Sicht vermittelt. Die Darstellung therapeutischer Verfahren umfasst u. a. die Techniken der Pleuradrainage und der Fibrinolysebehandlung beim Pleuraempyem sowie die Indikation und Durchführung der Pleurodese bei therapierefraktären bzw. malignen Ergüssen.
Abstract
Pleural effusions of unclear or uncertain origin represent a common clinical problem. A diagnostic thoracentesis and clinical chemistry allow to differentiate transudates from exudates. This classification helps to plan the further diagnostic and therapeutic approach. In the presence of a transudate, further diagnostic procedures in the pleural space are unneccessary. In this situation, the efforts should concentrate on the identification of one of the underlying diseases known to cause transudates. The presence of an exudate opens a wide range of differential diagnoses, mainly inflammatory or malignant disorders, but also other and rare conditions. The arsenal of diagnostic procedures in the pleural space includes cytology, bacteriology, blind pleural biopsy, conventional rigid pleuroscopy, and the innovative semiflexible pleuroscopy. This review includes a summary of the current and evidence-based concepts in the clinical management of parapneumonic effusions and empyemas, and of malignant effusions. The presentation of therapeutic procedures comprises a summary of the indications and techniques of drainage procedures, intrapleural fibrinolytic therapy, and of pleurodesis.
