Zusammenfassung
Die zystische Fibrose (CF) ist die häufigste tödlich verlaufende autosomal-rezessive Erkrankung in Europa. Die sichere frühzeitige Diagnose hilft, eine optimale Therapie einzuleiten; der Ausschluss einer CF ist u. U. für die Familienplanungen von großer Bedeutung. Die Diagnose ist nicht ausschließlich klinisch zu stellen. Es sind weitere Laboruntersuchungen notwendig. Die klassische Labordiagnostik ist weiterhin der Schweißtest. Erst wenn damit keine Diagnosesicherung gelingt, was in ca 5% der Fälle vorkommt, sind weitere Untersuchungen wie Genetik und Untersuchungen der Funktion des gestörten Chloridkanals (nasale Potenzialdifferenzmessung oder intestinale Kurzschlussstrommessung) nötig. Die Diagnostik der CF sollte möglichst in einer CF-Ambulanz durchgeführt werden. In den wenigen Fällen, in denen spezielle Untersuchungen notwendig sind, sollte mit den entsprechenden Einrichtungen frühzeitig Kontakt aufgenommen werden.
Abstract
Cystic fibrosis (CF) is the most frequent lethal autosomal recessive disease in Europe. An early and definite diagnosis is essential for an optimal therapy and the exclusion of CF might be important in the family planning. Diagnosis cannot be established only on the basis of clinical symptoms. Additional laboratory tests are mandatory to confirm the diagnosis. The classical test is the sweat test. If questions regarding the diagnosis remain, genetic testing or measurement of cystic fibrosis transmembrane conductance regulator (CFTR) functions (nasal potential difference or intestinal current measurement) are indicated. The CF centre is the place in which CF diagnostic tests should be initiated or carried out. In the few cases which need more sophisticated tests (e.g. nPD or ICM), the local CF centre should contact one of the centres which are familiar with these tests and send the patients there.
