Zusammenfassung
Tabakrauchen ist die Hauptursache für eine Vielzahl von Krankheiten, vorwiegend der Lunge. Umgekehrt gehört die Tabakabstinenz zu den effizientesten Präventivmaßnahmen in der Medizin. Während auf der Ebene der Primärprävention, also der Gesetzgebung, das stetige politische Engagement aller Ärzte gefragt ist, sind Pneumologen bei der Sekundärprävention, der Raucherentwöhnung, in Klinik und Praxis besonders gefordert. Bereits ein kurzer Ratschlag eines Arztes, ja die alleinige Frage nach den Rauchgewohnheiten, kann geeignet sein, den einen oder anderen Raucher zum Aufhören zu bewegen. Da die Raucherkarriere in verschiedenen Stadien verläuft, bedarf jeder Raucher eines anderen Zugangs und jeder Patient einer individuellen Betreuung. Zur Einschätzung der Abhängigkeitskomponente eines Rauchers kann der sog. „Fagerström-Test“ benutzt werden. Je nach Höhe des Ergebnisses lassen sich weitergehende Empfehlungen für die Art der Unterstützung und die Verwendung von medikamentösen Hilfen aussprechen. Intensive Gesprächstherapie plus Nikotinersatzprodukte oder Bupropion sind für eine dauerhafte Nikotinkarenz am erfolgversprechendsten. Eine Nachbetreuung der Nikotinkarenz sollte proaktiv erfolgen, wobei konkrete Hilfsangebote für den Fall eines Rückfalls Bestandteil sein sollten.
Abstract
Smoking is a major cause of lung disease as well as a number of other life-threatening diseases. Successful smoking cessation programs are therefore a highly efficient mode of disease prevention to succeed this aim at the level of primary prevention and legislation, the political engagement of all medical practitioners is necessary, whilst pneumologists are especially required for secondary prevention and the cessation of smoking. A simple question from the doctor about a patient on smoking habits or a recommendation to stop already may lead to a smoking cessation. Due to different stages on their way to quit, there is no single, simple strategy for supporting smokers. The “Fagerström-Test” may be a helpful tool for estimating nicotine addiction. According to the test result, it may be useful to suggest nicotine replacement therapy or other medication (e.g. bupropion) to support the smoker and prevent withdrawal symptoms. Motivational interview techniques plus nicotine replacement or bupropion are most promising to help. The follow-up of ex-smokers should be proactiv and precise instructions for them should be at hand to prevent any relapse.
