Zusammenfassung
Als Mediastinalemphysem bzw. Pneumomediastinum wird eine extraluminale Luftansammlung im Mediastinum bezeichnet. Pathogenetisch kommt eine Vielzahl intra- und extrathorakaler Erkrankungen in Frage, die das Krankheitsbild zu einer differenzialdiagnostischen Herausforderung machen. Es gilt, das im Regelfall harmlose spontane Mediastinalemphysem von einer mediastinalen Luftansammlung, verursacht durch potenziell lebensbedrohliche Erkrankungen, zu unterscheiden. Klinisch manifestiert sich das Mediastinalemphysem typischerweise mit einem akut einsetzenden thorakalen, retrosternalen Schmerz und mit Dyspnoe, seltener mit Husten, Hals- oder Rückenschmerzen, Hautemphysem, Dysphagie und Dysphonie. Die Inzidenz schwankt zwischen 1:12.000 und 1:30.000 für das spontane Auftreten eines Mediastinalemphysems und 1:320 bei thoraxchirurgischen Patienten. Dieser Beitrag möchte, basierend auf der aktuellen Literatur, eine Übersicht über Pathogenese, Diagnostik und Therapie dieses eher seltenen Krankheitsbildes geben.
Abstract
Mediastinal emphysema or pneumomediastinum is defined as extraluminal gas in the mediastinum. Pathophysiologically there exists a wide range of intra- and extrathoracic affections that make pneumomediastinum a diagnostic challenge. It is important to differentiate the mild and usual self-limiting spontaneous pneumomediastinum from underlying potential life-threatening affections. The typical clinical signs are chest pain and dyspnea, followed by coughing, sore and swollen throat or neck, dysphagia, and dysphonia. The incidence varies from 1:12,000 to 1:30,000 for spontaneous pneumomediastinum and 1:320 in thoracic surgical inpatients. Based on the literature this article would like to give a survey of the diagnosis, pathophysiology, and treatment of this rare but important clinical feature.
