Zusammenfassung
Die bildgebende Diagnostik des Herzens wird zunehmend von CT und MR dominiert. Die Mehrzeilen-Spiral-CT erlaubt eine quantitative und qualitative Analyse von Plaques der Koronargefäße, kontrastmittelgestützt auch ihres Lumens. Darstellung und Quantifizierung angeborener oder erworbener Herzerkrankungen bezüglich des Klappenapparates und des Myokards sind wegen des höheren Weichteilkontrastes im MR besser zu visualisieren. Der Perfusionsgrad und die Kontraktion des Myokards sowie die Volumetrie der Herzkammern können durch dynamische Sequenzen bestimmt werden. Bei der Bildgebung der Gefäßerkrankungen der Lunge ist zu unterscheiden zwischen den eher geläufigen Entitäten Lungenembolie und pulmonaler Hypertonus einerseits und dem heterogenen Spektrum meist angeborener sehr seltener Veränderungen der funktionellen und der nutritiven Strombahn. Bei Lungenembolie und pulmonalem Hypertonus hat das CT den höchsten diagnostischen bzw. differenzialdiagnostischen Stellenwert. Für die Gruppe der oft komplexen seltenen Gefäßerkrankungen hat das Röntgensummationsbild eine Schlüsselfunktion, hier finden sich häufig diagnostische Wegweiser. Die endgültige Abklärung erfolgt dann computertomographisch, zur Analyse der systemischen Gefäße angiographisch.
Abstract
Computed tomography (CT) and magnetic resonance imaging (MRI) are playing an increasingly important role in the diagnosis of cardiac diseases. Multidetector row spiral CT is capable of quantifying coronary vessel disease by means of detecting calcified plaques and visualizing the degree of coronary stenosis with the use of i.v. contrast. Due to its capability for tissue specific and flow dependent contrast, MRI is superior to CT in imaging valvular and myocardial disease processes. In addition, modern post-processing tools allow flow and velocity as well as volumetric measurements and visualisation of myocardial contractility. Pulmonary artery (PA) embolism and PA hypertension are much more common indications for radiologic imaging than the quite heterogenic spectrum of mostly inborn pulmonary vascular diseases. CT is the tool of choice for the diagnosis of PA embolism and the differential diagnosis of hypertension. In the latter group, standard chest radiography is a diagnostic primer which reveals, in most cases, the diagnosis. Digital subtraction angiography in addition is helpful in judging changes to the systemic vasculature.
