Zusammenfassung
Der diagnostische Aufwand beim akuten Husten beschränkt sich in der Regel auf eine sorgfältige Anamnese und eine gründliche Untersuchung. Therapeutisch sind bei der häufigsten Ursache des akuten Hustens, dem akuten, viralen Atemwegsinfekt, Hausmittel zumeist ausreichend. Die häufigste Ursache des chronischen Hustens ist das Rauchen, 4–6 Wochen nach Tabakabstinenz ist der Exraucher symptomfrei. Beim Nichtraucher wird chronischer Husten überwiegend durch Asthma bronchiale, gastroösophagealen Reflux und chronische Rhinosinusitis ausgelöst. Zur Abklärung des chronischen Hustens bedient man sich eines Algorithmus, der auf der klinischen Verdachtsdiagnose aufbaut und sich im diagnostischen Ablauf an der Häufigkeit der Ursachen ausrichtet. In der überwiegenden Mehrzahl der Fälle führt die auf diesen Erkenntnissen basierende Therapie zu Beschwerdefreiheit. Eine rein symptomatische Therapie mit Antitussiva ist erst nach sorgfältiger Abklärung und nur bei quälendem, unproduktivem Husten angezeigt.
Abstract
In the work-up of acute cough, usually a carefully taken history and a complete physical examination will be diagnostic. For the treatment of the most common cause of acute cough, namely, acute viral respiratory infection, special treatment will not be necessary. The most common cause of chronic cough is smoking; within 4–6 weeks after abstaining from tobacco, the ex-smoker will become free of symptoms. In non-smokers, the most common causes of chronic cough are asthma, gastroesophageal reflux, and rhinosinusitis. In the diagnostic work-up of chronic cough, one should follow an algorithm, which is based on the patient’s history and the most common causes of chronic cough. In most cases, the treatment based on this algorithm will be successful. Symptomatic treatment with antitussive drugs is only recommended after careful examination, if the cough is distressing and nonproductive.
