Zusammenfassung
Die Lungentransplantation ist ein Therapieverfahren für Patienten in Endstadien von Lungenerkrankungen. Häufig zugrunde liegende Krankheitsbilder sind die zystische Fibrose, die Lungenfibrose und das Lungenemphysem. Wichtige Kontraindikationen stellen unkontrollierbare Infektionsherde, schwere kardiovaskuläre Begleiterkrankungen, extremes Über- oder Untergewicht oder fortgeschrittene muskuläre Dekonditionierung dar. Potenzielle Kandidaten und das Verfahren (einseitige, doppelseitige oder Herz-Lungen-Transplantation) müssen im Sinne einer Nutzen-Risiko-Abschätzung sorgfältig ausgewählt werden. Die Langzeitergebnisse der Lungentransplantation liegen noch hinter denen der Übertragungsverfahren anderer solider Organe zurück. Nach 5 Jahren können inzwischen Überlebensraten von ca. 60% erreicht werden.
Abstract
Lung transplantation is an option for patients with end-stage pulmonary diseases without contraindications. Common underlying diseases are cystic fibrosis, idiopathic pulmonary fibrosis and emphysema. Important contraindications are uncontrollable infectious sources, severe cardiovascular problems, morbid obesity and cachexia or advanced muscular deconditioning. Selection of transplant candidates and the surgical procedure (single, double or heart-lung-transplantation) is essential to provide a reasonable risk-benefit ratio. Life expectancy after lung transplantation is still shorter than in patients with other solid organ transplantations. A 5-year survival after lung transplantation averages, in some centers, 60%.
