Zusammenfassung
Definitionsgemäß handelt es sich bei einem isolierten Lungenrundherd („solitary pulmonary nodule“, SPN) um einen rundlichen Tumor im Lungenparenchym mit einem Durchmesser <3 cm. Pro 500 Röntgenbilder der Thoraxorgane wird ein SPN als Zufallsbefund beobachtet. Die Inzidenz maligner Veränderungen variiert zwischen 10 und 70%. Für die Diagnostik eines SPN sollte ein Algorithmus zur Anwendung kommen, der die Malignomwahrscheinlichkeit berücksichtigt. Diese Wahrscheinlichkeit ist von der Größe des Rundherdes, der Anamnese bezüglich des inhalativen Nikotinkonsums, dem Patientenalter und den Charakteristika des Rundherdes im CT abhängig. Ist keine Größenänderung über einen Zeitraum von 2 Jahren anhand von Röntgenbildern zu dokumentieren, ist keine weitere Diagnostik notwendig. Ist die Wahrscheinlichkeit für ein Malignom niedrig, sollte ein Monitoring mit seriellen HR-CTs erfolgen. Dagegen ist bei hoher Malignomwahrscheinlichkeit die chirurgische Resektion unter der Voraussetzung eines akzeptablen Operationsrisikos indiziert. Bei einer intermediären Wahrscheinlichkeit (10–80%) ist eine weiterführende Diagnostik sinnvoll. All diese Überlegungen werden in Diskussion mit dem aufgeklärten Patienten realisiert.
Abstract
By definition, a solitary pulmonary nodule (SPN) is a round tumor in the lung parenchyma with a diameter <3 cm. For every 500 X-rays taken of the thoracic organs, 1 case of SPN is discovered incidently. The incidence of malignant changes varies between 10 and 70%. An algorithm that takes into account the probability of malignant tumors should be applied in the diagnosis of SPN. Probability depends on the size of the nodule, that patient’s history of inhaling smoke, the patient’s age, and the characteristics of the nodule as visualized by computed tomography. If no change is documented in X-ray images over a span of 2 years, then no further diagnostic tests are necessary. If the probability of a malignant tumor is low, the further course should be monitored with serial HRCTs. If, in contrast, the probability of malignancy is high, surgical resection is indicated provided that the surgical risk is acceptable. In cases of intermediate probability (10–80%), additional diagnostic assessment is judicious. All of these considerations should be thoroughly discussed with the informed patient.
