Zusammenfassung

Im Rahmen der präoperativen PatientInnenevaluation erhebt die Anästhesistin/der Anästhesist die Blutungsanamnese, sowie Risikofaktoren für thrombotische Komplikationen und initiiert eine individuelle Gerinnungsoptimierung. Neue Antikoagulantien können den Blutverlust und das Risiko spinaler Hämatome steigern – der postoperative Beginn der Thromboseprophylaxe und Empfehlungen zum Zeitpunkt von blutungsriskanten Interventionen relativ zur Einnahme der Gerinnungshemmer helfen das Risiko zu minimieren. Mit der Zulassung in kardiologischen Indikationsgebieten werden wir mit Patienten unter Dauertherapie mit neuen Antikoagulanzien konfrontiert werden. Bis dahin sollten wir offene Fragen klären, z. B. zur Repetition der Thromboseprophylaxe bei postoperativem Erbrechen oder zu Interaktionen mit der postoperativen Schmerztherapie.

Summary

During preoperative patient evaluation, anaesthesiologists assess the patient's bleeding history and risks for thrombosis and initiate individualized coagulation management. New potent anticoagulants may increase blood loss and the risk for spinal haematoma in patients scheduled for neuraxial anaesthesia. Postoperative start of thromboprophylaxis and recommendations on the timing of invasive interventions help in controlling these risks. Before widespread use for cardiological indications open questions need to be answered e.g. oral drug administration in postoperative vomiting and potential interactions with postoperative pain therapy.
