Doctor Shopping: Über den Umgang mit schwierigen Patientinnen und Patienten

Medically unexplained symptoms and bodily dysfunctions are the single most prevalent class of symptoms in primary care. Differences in opinion as to the cause of the illness between the patients and the health care professionals make somatoform disorders a challenge for the patient-doctor relationship, and frequently produce negative feelings in health care professionals. The patients are usually reluctant to consider psychosocial aspects of their illness to be a factor. Early recognition of patients with somatization tendencies would be beneficial so that more effective treatment could begin sooner and unnecessary treatment be avoided. In order not to reinforce the patient's symptomatology, physicians should schedule appointments in a time contingent rather than a symptom contingent manner. Repetition of different tests should be avoided despite patients' demands in order not to further exacerbate the disorder. In many patients, psychotherapy should be recommended, although motivational work should already be initiated at the primary care stage.

Zusammenfassung

Somatoforme Störungen kommen in der Primärversorgung ausgesprochen häufig vor und stellen aufgrund unterschiedlicher Ursachenüberzeugungen eine Herausforderung für die Arzt-Patient-Beziehung dar. Die Idealisierung, nachfolgende Enttäuschung und schließlich der Arztwechsel werden durch Begriffe wie "hospital hopping", "doctor shopping" oder "Arztnomaden" beschrieben. Bei Vorliegen negativer organischer Befunde sollte frühzeitig angesprochen werden, dass mögliche psychosomatische Zusammenhänge vorliegen könnten. Wichtig ist eine Entkoppelung von Kontaktangeboten und Beschwerdeintensität durch die Vereinbarung zeitkontingenter anstatt symptomkontingenter Termine. Mit Wiederholungen von Untersuchungen sollte trotz Drängen der Patientinnen und Patienten vorsichtig umgegangen werden um nicht zu einer weiteren Chronifizierung beizutragen. In vielen Fällen sollte eine Weiterleitung zur Psychotherapie empfohlen werden, wobei bereits in der Primärversorgung Motivationsarbeit geleistet werden muss.
