Subacute presentation of spontaneous diaphragmal rupture: a case report

Summary

BACKGROUND: Diaphragmal injury is a common and potentially dangerous condition. It sometimes presents with delay and can cause strangulation and perforation of abdominal viscera. METHODS: We present a case report of a 14-year-old boy who presented with left lumbar pain and developed peritonitis six days after strenuous training. RESULTS: Incidence, pathogenesis, and diagnostic pitfalls of diaphragmal injury are described briefly. Case history, diagnostic procedures, and treatment are described. Spontaneous diaphragmal rupture is discussed. CONCLUSIONS: We stress the need to seek the condition actively and to keep a low threshold for a chest X ray in patients who present with any kind of pain in the thoracoabdominal region after trauma or endogenous strain. Diaphragmal ruptures and hernias have a low morbidity and mortality when treated on time but can progress to disaster quickly if missed.

Zusammenfassung

GRUNDLAGEN: Zwerchfellverletzungen sind eine nicht selten vorkommende gefährliche Situation. Nicht selten wird sie erst verspätet symptomatisch und kann zu Darmstrangulation und Perforation führen. METHODIK: Fallbericht eines 14-jährigen Knaben mit links lumbalen Schmerzen und Peritonitis 6 Tage nach Training. ERGEBNISSE: Inzidenz, Pathogenese und diagnostische Schwierigkeiten einer Zwerchfellverletzung werden dargestellt. Fallbericht, Diagnostik und Behandlung der Zwerchfellruptur werden beschrieben. SCHLUSSFOLGERUNGEN: Bei postraumatischen thorakalen Schmerzen sollte immer an eine Zwerchfellruptur gedacht werden. Man sollte nicht zögern, ein Thorax-Röntgen machen zu lassen. Zwerchfellrupturen haben eine niedrige Morbidität und Mortalität, wenn sie rechtzeitig erkannt und therapiert werden, können aber schwerwiegende Folgen haben, wenn sie übersehen werden.
