Muscle balance: An important factor for nerve regeneration

BACKGROUND: It is generally known that the prognosis of peroneus nerve lesions is much worse when compared to other nerves. One of the reasons for this fact might be the imbalance between the peroneus-innervated muscles of the anterior compartment and the much stronger muscles of the calve. METHODS: Transferring the tibialis posterior muscle to the dorsum of the foot. RESULTS: This transfer did not only provide a motor for extension of the ankle joint but stimulated a significantly improved regeneration of all the muscles of the anterior compartment. CONCLUSIONS: Establishing a better balance between antagonistic muscles seems to be an important factor in functional recovery.

Zusammenfassung

GRUNDLAGEN: Es ist allgemein bekannt, dass die Prognose von Läsionen des Nervus peroneus wesentlich schlechter ist als bei anderen Nerven. Ein möglicher Grund könnte die Tatsache des Ungleichgewichtes zwischen den vom N. peroneus innervierten Muskeln der vorderen Loge und den Muskeln der Wade sein. METHODIK: Eine Transposition des Tibialis-posterior-Muskels zum Fußrücken. ERGEBNISSE: Diese Transposition bedeutet nicht nur das Herbeischaffen eines Motors für die Streckung des Sprunggelenkes, sondern stimuliert auch das Fortschreiten der Regeneration aller Muskeln der vorderen Loge. SCHLUSSFOLGERUNGEN: Die Wiederherstellung eines Muskelgleichgewichtes zwischen antagonistischer Muskulatur scheint ein wichtiger Faktor in Hinsicht auf eine bessere Funktion zu sein.
