Early results of laparoscopic adjustable gastric banding using the new low-pressure Soft Gastric Band^®

BACKGROUND: Gastric banding represents the most commonly performed restrictive bariatric procedure nowadays in Europe and Australia. In order to reduce the rate of band migration through the gastric wall, different types of low-pressure bands have recently been developed. The aim of this study was to evaluate the safety and efficacy of a new type of low-pressure gastric band. METHODS: Within a period of 18 months the Soft Gastric Band^® was implanted in 60 morbidly obese patients (female, 43; male, 17; mean age, 38.4 years; range, 14.5–63.3 years). In all patients, implantation was performed using the "pars flaccida" technique, all procedures were completed laparoscopically. The average duration of surgery was 80 ± 30 minutes. RESULTS: Mean body mass index at time of surgery was 48.1 ± 6.7 kg/m²; mean body weight was 140 ± 26 kg. After a median follow-up of 12 months in 93% of the patients an average excessive-weight loss at 6, 12, 18 and 24 months of 26.8% ± 11.4%, 36.2% ± 16.7%, 45.0% ± 18.0% and 49.0% ± 37.1%, respectively, was achieved. This corresponds to the results of other band types. Complications consisted in port occlusion in three patients and abscess formation at the port site in one patient. In one patient, re-banding was performed due to band leakage 42 months postoperatively. Up to now, neither band migration nor pouch enlargement or slipping was observed during follow-up. CONCLUSIONS: Gastric banding using the Soft Gastric Band^® is a safe and effective procedure in bariatric surgery. No band migration through the gastric wall was observed. A longer follow-up has to be achieved to point out the real incidence of long-term complications.

Zusammenfassung

GRUNDLAGEN: Die Anlage eines verstellbaren Magenbandes stellt derzeit die am häufigsten durchgeführte restriktive bariatrische Operation in Europa und Australien dar. Verschiedene Niedrigdruckbänder wurden entwickelt, um die Rate an Bandmigrationen durch die Magenwand zu verringern. Ziel dieser Studie war es, die Sicherheit und Effektivität eines neuen solchen Niedrigdruckbandes zu untersuchen. METHODIK: Über einen Zeitraum von 18 Monaten wurden bei 60 krankhaft adipösen Patienten (weiblich, 43; männlich, 17; durchschnittliches Alter, 38,4 Jahre; Bereich, 14,5– 63,3 Jahre) das Soft Gastric Band^® in der "pars flaccida"-Technik laparoskopisch implantiert. Die durchschnittliche Operationsdauer betrug 80 ± 30 Minuten. ERGEBNISSE: Der mittlere Body Mass Index zum Zeitpunkt der Operation betrug 48,1 ± 6,7 kg/m², bei einem mittleren Körpergewicht von 140 ± 26 kg. Nach einem medianen Nachbeobachtungszeitraum von 12 Monaten bei 93 % der Patienten konnte ein mittlerer Excessive Weight Loss von 26,8 % ± 11,4 % nach 6, 36,2 % ± 16,7 % nach 12, 45,0 % ± 18,0 % nach 18 und 49,0 % ± 37,1 % nach 24 Monaten erreicht werden. Diese Ergebnisse entsprechen somit denen anderer Bandsysteme. Als Komplikationen wurden 3 Portverschlüsse sowie ein Abszess im Portbereich beobachtet. In einem Fall musste aufgrund einer Band-Leakage ein Bandwechsel 42 Monate postoperativ durchgeführt werden. Bis zum jetzigen Zeitpunkt wurden weder Bandmigration noch Pouch-Enlargement oder Slipping beobachtet. SCHLUSSFOLGERUNGEN: Die Verwendung des Soft Gastric Band^® stellt eine sichere und effektive Methode zur Anlage eines Magenbandes dar. Bislang wurden keine Bandmigrationen durch die Magenwand beobachtet, ein längerer Nachbeobachtungszeitraum bleibt jedoch diesbezüglich abzuwarten.
