Summary
BACKGROUND: Perforated mycotic aortic aneurysms remain a major problem in vascular surgery. These patients continue to present a challenge to both the vascular surgeon and the medical microbiologist, although improvements in graft materials, transplant surgery and perioperative management have led to decreased mortality. METHODS: The case report summarizes the medical history of a 44-year-old male treated at our hospital for perforated Salmonella mycotic aneurysm and recurrent septic episodes. RESULTS: The patient suffered from recurrent septic episodes originating in the oral cavity and a duodenal fistula for more than five years following an alloplastic aortobiiliacal bypass. The pathogenesis, diagnostic approach and management of this case are discussed. CONCLUSIONS: Duodenal fistulae following aortic graft implantation are a rare, but serious complication, which will cause septic episodes and graft infection. This case demonstrates that a careful identification of the involved pathogens may aid the identification of the infectious focus.
Zusammenfassung
GRUNDLAGEN: Obwohl verbesserte alloplastische Prothesen, die Transplantationschirurgie und ein besseres perioperatives Management zu einer Verringerung der Mortalität geführt haben, stellen perforierte mykotische aortale Aneurysmata weiterhin ein Problem in der Gefäßchirurgie dar. Sie bedürfen einer engen Zusammenarbeit des Gefäßchirurgen und Mikrobiologen. METHODIK: Die vorliegende Falldarstellung fasst die Krankengeschichte eines 44-jährigen Patienten zusammen, der an unserer Klinik wegen eines perforierten mykotischen (Salmonellen) Aneurysma und rezidivierender septischer Episoden behandelt wurde. ERGEBNISSE: Nach alloplastischer Gefäßrekonstruktion litt der Patient über fünf Jahre an rezidivierenden septischen Episoden, ausgehend von einem oralen Fokus und einer duodenalen Fistel. Es werden die Pathogenese, das diagnostische Vorgehen und die weitere Therapie dargestellt. SCHLUSSFOLGERUNGEN: Duodenale Fisteln nach Aortenersatz sind seltene, jedoch ernste Komplikationen, welche eine Sepsis und eine Infektion der Gefäßprothese verursachen können. Der vorliegende Fall demonstriert, dass eine sorgfältige Identifikation der gefundenen Erreger zu der Infektionsquelle führen kann.
