Riassunto
Il trattamento chirurgico a cielo aperto (ORIF) delle fratture del pilone tibiale è gravato da una serie di complicanze in termini di infezioni e controllo dei tessuti molli. La fissazione esterna attraverso la ligamentotassi, con o senza procedure mininvasive, rappresenta una metodica alternativa, ma che garantisce risultati funzionalmente paragonabili. Nel periodo compreso dal 2001 al 2010, presso la Divisione di Traumatologia dell’ospedale CTO di Torino, sono state trattate 90 fratture del pilone tibiale in 90 pazienti, utilizzando il fissatore esterno circolare. Il tempo medio di guarigione è stato di 26 settimane. Le complicanze avute sono state: 1 caso di infezione profonda (risolto con sequestrectomia); 20% di flogosi superficiale (risolti con terapia antibiotica orale e antinfiammatoria); 3 dei 90 pazienti hanno avuto una artrodesi. La rimozione del fissatore è avvenuta mediamente a 26 settimane. Il risultato clinico è stato buono nella maggior parte dei pazienti; non c’è stata sempre correlazione tra l’entità della riduzione e i risultati funzionali, avendo avuto buoni risultati funzionali anche in situazioni di scadente risultato radiologico.
Abstract
Open reduction and internal fixation of tibial pilon fractures is burdened with high risks of infections and soft tissue complications. The circular external frame, by means of the ligamentotaxis and allowing for minimally invasive procedures into the joint, could be an effective option. Functional outcomes are encouraging. In the 2001-2010 period 90 fractures in 90 patients were operated in our institution with the circular external frame. In this study the time of frame removal has been considered as the healing time: on average it has been 26 weeks. We report 18 (20%) minor complications (superficial pin infections, treated with oral antibiotics and NSAIDs) and 4 (4.4%) major problems (1 deep infection treated with a sequestrectomy and 3 severe postraumatic arthritis in which an arthrodesis was performed). On average the functional result has been good. We have observed a weak association between X-rays and clinical results, where some unsatisfactory radiographs corresponded with sufficient clinical results (good painless ROM or with minor pain).
