Riassunto
Le fratture sovracondiloidee di omero sono le più comuni lesioni del gomito in età pediatrica. Le fratture di tipo I secondo Garland sono trattate incruentemente, mentre quelle di tipo II e III richiedono trattamento chirurgico. Le fratture di tipo II sono trattate con riduzione a cielo chiuso e fissazione. Le fratture tipo III possono richiedere la riduzione a cielo aperto.
Dal 2001 al 2011 abbiamo trattato chirurgicamente 44 pazienti, 8 con fratture tipo II e 35 con fratture tipo III. Tutti i pazienti sono stati stabilizzati con 2 fili di Kirschner. Le fratture di tipo II sono state ridotte a cielo chiuso, quelle di tipo III a cielo aperto. Sono stati riscontrati: un caso di disturbi a carico del nervo ulnare e un caso a carico del nervo radiale, regrediti spontaneamente, un caso di lieve deformità in varo e uno in valgo. In nessuna evenienza è stato necessario un nuovo intervento chirurgico.
Abstract
The supracondylar fracture of the distal humerus is the most common pediatric fracture in the elbow. Gartland Type I fractures are managed non-surgically, but types II and III require surgical intervention. Standard treatment for type II is closed reduction and percutaneous pinning. Type III fractures may require open reduction.
From 2001 to 2011 we surgically treated 44 patients, 8 with type II fractures and 35 with type III fractures. All patients were stabilized with two pins. We used closed reduction for type II fractures and open reduction for type III. There were one ulnar nerve injury and one radial nerve injury spontaneously regressed. Two patients had pin-track infection requiring antibiotic therapy. One patient had cubitus varus and one patient cubitus valgus. No patient required additional surgery.
