Zusammenfassung
Bei ausgedehnter Pathologie des Frontallappens (meist Malformationen kortikaler Entwicklung) ist eine insuläre Mitbeteiligung häufig. Eine vollständige Mitresektion des frontoinsulären Übergangs und des rostralen Inselkortex ist nach Resektion des Frontallappens möglich. Ausgedehnte Eingriffe im dominanten Frontallappen sind sicher durch Resektion unter Schonung des anatomischen Broca-Areals und Fasciculus arcuatus zu verwirklichen. Resektionen des Frontallappens sind bei nicht eröffnetem Ventrikelsystem komplikationsarm und haben gegenüber diskonnektiven Techniken keine objektiven Nachteile. Bei frontalen Pathologien die keine komplizierenden Faktoren aufweisen (Beteiligung Inselkortex, dominante Hemisphäre, Mitbeteiligung der Zentralregion), stellt die diskonnektive Technik eine Operationsvariante mit vergleichbaren Risiken dar.
Abstract
In cases with extensive pathology of the frontal lobe (mostly malformations of cortical development) insular involvement is common. Complete coresection of frontoinsular transition and rostral insular cortex is possible after resection of the frontal lobe. Extensive procedures in the dominant frontal lobe can be safely accomplished by resection while sparing the anatomic Broca’s area and fasciculus arcuatus. Resections of the frontal lobe have few complications when the ventricular system is not opened and have no objective disadvantages compared with disconnective techniques. In frontal pathologies that do not have complicating factors (involvement of insular cortex, dominant hemisphere, involvement of the central region), the disconnective technique is an elegant surgical option.
