Neurological differential diagnosis of paroxysmal vomiting: gastrointestinal cause in familial cavernomatosis

Zusammenfassung

Paroxysmalem Erbrechen und anderen paroxysmal auftretenden gastrointestinalen Beschwerden kann eine epileptische Genese zugrunde liegen. Patienten mit einer solchen Fragestellung werden bei Vorliegen zerebraler, insbesondere temporaler Läsionen, und unauffälliger internistischer Zusatzdiagnostik häufiger zur differenzialdiagnostischen Einschätzung zugewiesen. Die Aufzeichnung habitueller Episoden im Video-EEG-Monitoring kann bei der Eingrenzung der Differenzialdiagnosen behilflich sein, auch wenn die Diagnosesicherung wie in dem geschilderten Fall erst durch einen Ösophagusbreischluck möglich war.

Abstract

Paroxysmal vomiting and other paroxysmal gastrointestinal symptoms can have an underlying epileptic origin. Patients with such symptoms are referred for a differential diagnostic work-up in the presence of cerebral, in particular temporal lesions and inconspicuous internal medical additional diagnostics. The recording of habitual episodes during video electroencephalography (EEG) monitoring can help to narrow down the differential diagnosis, even if confirmation of the diagnosis was only possible by a barium swallow X‑ray, as in the case described here.
