Zusammenfassung
Bedarfsmedikamente sind ein wichtiges Instrument zur Unterbrechung von Anfallsserien und zur initialen Behandlung eines Status epilepticus. In den letzten Jahren hat sich allerdings eine Entwicklung in der Indikationsstellung zur Akutbehandlung von epileptischen Anfällen ergeben, die häufig nicht mehr sachgerecht ist, da die intendierten Ziele aufgrund der pharmakologischen Gegebenheiten gar nicht erreichbar sind. Neben den Strategien einer i.v.-Verabreichung von Medikamenten, die kurz dargestellt werden, hat sich ein breiter Einsatz in der eine Anwendung insbesondere von Benzodiazepinen rektal, sublingual/bukkal und nasal entwickelt. Die vorhandenen pharmakokinetischen Daten werden referiert. Beim Status epilepticus ist eine i.v.-Behandlung erforderlich, die durch rektale, bukkale oder intramuskuläre Gaben nur flankiert werden kann. Bei Anfallsserien und dadurch bedingter Erschöpfung der Patientin kann mit einer rektalen Gabe von Diazepam eine Wirkung innerhalb von Minuten, durch eine bukkale Gabe von Midazolam eine kurze Wirkung innerhalb von 5–10 min und durch sublinguale Gabe von Lorazepam eine anhaltende, aber erst nach 30 min einsetzende Wirkung erreicht werden. Der einzelne epileptische Anfall ist nur in Ausnahmefällen eine Indikation für eine Akutbehandlung.
Abstract
Acute medications are important instruments for interruption of a series of seizures and for initial treatment of a status epilepticus; however, in the past few years there has been a development in the indications for acute therapy of seizures, which is often no longer appropriate because the intended target is not achievable due to the pharmacological conditions. In addition to the strategies for intravenous administration of medications, which are briefly described, a broad approach especially of rectal, sublingual/buccal and nasal administration of benzodiazepines has developed. The available pharmacokinetic data are reviewed. In cases of status epilepticus an intravenous treatment is necessary, which can only be flanked by rectal, buccal or intramuscular administration. For serial seizures resulting in exhaustion of the patient, rectal administration of diazepam can be effective within minutes, buccal administration of midazolam can have a brief effect of 5–10 min and sublingual administration of lorazepam can achieve a persistent effect but only after a delay of 30 min. Isolated epileptic seizures are an indication for acute medicinal treatment only in exceptional cases.
