Zusammenfassung
Bei Kindern mit Zerebralparese (CP) stellt eine Epilepsie eine wesentliche Komorbidität dar; sie tritt bei etwa einem Drittel der Betroffenen auf und ist mit der Schwere der Behinderung korreliert. Da die CP eine vorwiegend läsionelle Gehirnerkrankung ist, lässt dies auch annehmen, dass die Epilepsie symptomatisch ist und mit der Schwere der Gehirnschädigung korreliert. Auch Gehirnfehlbildungen sind häufiger mit einer Epilepsie assoziiert. Um auf bevölkerungsbezogener Ebene analysieren zu können, welche Rolle Läsionstyp und -topographie für das Auftreten und die Schwere einer Epilepsie spielen, ist neben der standardisierten Erhebung von Bildgebungsbefunden des Gehirns die differenziertere Erfassung nicht nur zum Auftreten, sondern auch zur Schwere einer Epilepsie bei der CP notwendig. Die Surveillance of Cerebral Palsy in Europe (SCPE) übernimmt eine Definition, die sich am Alter des Auftretens als Parameter für die Schwere orientiert. Für Register, die sich hauptsächlich auf medizinische Unterlagen berufen müssen, sind dies einfache und zuverlässige Parameter, die damit auch eine erschöpfende Datenerfassung erlauben.
Abstract
For children with cerebral palsy (CP), epilepsy is a significant comorbidity; it affects about one third of this patient group and correlates with the severity of the disability. As CP is a predominantly lesional brain disorder, the assumption can be made that epilepsy is symptomatic in most cases and the severity of brain lesion is linked to epilepsy. Brain malformations are also more often associated with epilepsy. For a population-based analysis of the role of lesion type and topography regarding the occurrence and severity of epilepsy, a detailed definition is necessary in addition to the standardized recording of neuroimaging findings. The Surveillance of Cerebral Palsy in Europe (SCPE) agrees on a definition based on age at onset of epilepsy as an indication of severity. For registers relying mainly on medical records, this provides a simple, reliable, and thus also comprehensive approach.
