Zusammenfassung
In dieser Sonderausgabe der Zeitschrift für Epileptologie werden optimierte Protokolle für die Durchführung klinischer Bildgebungsuntersuchungen beschrieben. Einzelfallbeschreibungen und Gruppenanalysen struktureller und funktioneller Studien sowie Studien zur Konnektivität weisen auf besondere anatomische Strukturen und Netzwerke hin, die für die Modulation von Anfallstätigkeit von Bedeutung sind. Während fokale Epilepsien von kortikal-subkortikalen Netzwerken abhängig sind, spielt die supplementär motorische Region eine besondere Rolle für die Anfallsauslösung bei juveniler myoklonischer Epilepsie. Neben der etablierten Bedeutung der Bildgebung für die Indikation und Prognose von Epilepsiechirurgie ist eine wichtige Herausforderung der zukünftigen Nutzung der Bildgebung die Prädiktion der medikamentösen Therapieantwort.
Abstract
In this special edition of Zeitschrift für Epileptologie, optimized protocols for clinical imaging studies are described. Exemplary cases and group analyses of functional, structural and effective connectivity provide evidence for unique anatomical structures that are involved in the modulation of seizure activity, and their role in specific syndromes. Whilst there is evidence that focal seizures arise and are sustained within a network of interconnected brain regions, altered connectivity with the supplementary motor area may represent the anatomical basis for cognitive triggering of motor seizures in juvenile myoclonic epilepsy. A major challenge for the future use of neuroimaging is the widening of applications beyond surgical imaging to include prediction of response to medication.
