Abstract
The group of infantile and neonatal seizures belongs to the idiopathic focal epilepsies. It comprises of benign familial neonatal seizures (BFNS), benign familial infantile seizures (BFIS) and benign familial neonatal/infantile seizures (BFNIS). These syndromes are characterized by a defined onset period and the clinical manifestation with singular or clustered complex partial or generalized seizures. The main differences are the seizure onset and the genetic and pathophysiological background. Whereas BFNS starts at the age of 1 to 3 postnatal days and ends spontaneously after a few weeks or months, BFIS begins at the age of 3 to 12 months. The onset of BFNIS lies in between BFNS and BFIS, so that seizures can occur in the neonatal and early infantile period.
Up to now, two genes have been identified for BFNS, KCNQ2on chromosome 20q13 and KCNQ3on chromosome 8q24, coding for subunits of voltagegated potassium channels. BFNIS is caused by mutations in SCN2Aon chromosome 2q23-q24.3, a gene encoding for a neuronal voltage-gated sodium channel. BFIS is linked to two loci which are localized on chromosome 19q12-13.11 and chromosome 16p12-q12 but a responsible gene has still not been detected.
This article describes the clinical characteristics, genetics and pathophysiology of benign neonatal and infantile seizures. In addition, the article gives an overview how research in such genetic epilepsy syndromes may help to improve the therapy of epilepsies and other neurological diseases which are based on membrane instability.
Zusammenfassung
Die Gruppe der benignen Anfälle des Neugeborenen- und Säuglingsalters gehört zu den idiopathischen fokalen Epilepsien. Sie umfasst drei Syndrome: die benignen familiären neonatalen (BFNS), die benignen familiären infantilen (BFIS) und die benignen familiären/neonatalen Anfälle (BFNIS). Die Syndrome zeichnen sich durch Anfälle in einer besonderen Lebenszeitspanne aus, die einzeln oder als Cluster von komplex-fokalen oder generalisierten Anfällen auftreten. Die Unterschiede der drei Formen betreffen das Alter bei Beginn der Anfälle, die genetische Grundlage und die damit verbundene unterschiedliche Pathophysiologie. BFNS beginnen zwischen dem 1. und dem 3. Lebenstag und sistieren nach ein paar Lebenswochen bis -monaten, BFIS kommen im Alter von 3 bis 12 Monaten vor und das Alter bei Beginn der BFNIS liegt dazwischen, so dass sowohl ein neonataler als auch infantiler Beginn möglich ist.
Bisher wurden zwei Gene für BFNS identifiziert, KCNQ2auf Chromosom 20q13 und KCNQ3auf Chromosom 8q24, die beide für Untereinheiten von spannungsgesteuerten Kaliumkanälen kodieren. BFNIS werden durch Mutationen in SCN2Aauf Chromosom 2q23-q24.3 verursacht, das Gen eines spannungsgesteuerten Natriumkanals. Für BFIS wurden Kopplungen zu zwei Genloci entdeckt, zu den Chromosomenabschnitten 19q12-13.11 und 16p12-q12. Die für dieses Syndrom verantwortlichen Gene konnten bislang noch nicht identifiziert werden.
Dieser Artikel beschreibt die klinischen Charakteristika, die Genetik und die Pathophysiologie erblicher Epilepsiesyndrome am Beispiel der benignen neonatalen und infantilen Anfälle. Er zeigt zudem Perspektiven auf, wie Forschungsarbeiten zu derartigen Syndromen dazu beitragen können, die Therapie von Epilepsien und anderen neurologischen Erkrankungen zu verbessern, die mit einer Membraninstabilität einhergehen.
