Abstract
Manifestation of neurally mediated syncope may cause driver incapacity. German drivers regulations comment on arrhythmias and orthostatic hypotension as causes of syncope. No advice is given for neurally mediated, especially frequently occurring vasovagal syncopes.
Data suggest that the medical condition of a driver is not an important factor in road accidents. The evidence in favor of drug therapy especially for vasovagal syncope remains unconvincing. Tilt testing to assess any therapy has no predictive value. There is no evidence that allowing three asymptomatic months to elapse provides any confirmation that syncope will not recur. The board of the European Society of Cardiology (ESC) modified the recommendations of the Task Force Report on driving in 2004. For group 1 drivers, the ESC advises minimal restrictions. In case of severe syncope (very frequent or occurring unpredictably or while performing a highrisk activity) symptom control is advised for group 1 drivers and permanent restriction unless effective treatment has been established for group 2 drivers.
Zusammenfassung
Die Manifestation neurokardiogener Synkopen (zu denen die vasovagalen Synkopen sowie situationsgebundene Reflexsynkopen und das Carotis-Sinus-Syndrom zählen) wirft die Frage nach der Fahreignung auf. Die „Begutachtungs-Leitlinien für die Kraftfahrereignung“ nehmen jedoch allein zu Synkopen im Zusammenhang mit Herzrhythmusstörungen und arterieller Hypotonie Stellung.
Die Beurteilung der Fahreignung bei Synkopen erfolgt vor dem Hintergrund, dass medizinische Ursachen für Unfälle insgesamt selten sind. Insbesondere für die häufigen vasovagalen Synkopen existiert weder eine evidenzbasierte Therapie noch eine evaluierte Therapiekontrolle mit prädiktivem Wert. Auch existieren keine epidemiologischen Daten, die erkennen ließen, dass nach einer Beobachtungszeit von drei Monaten, in der der Patient asymptomatisch blieb, das Rezidivrisiko in der Folge gemindert wäre. Belegt ist nur das erhöhte Rezidivrisiko nach häufigen Synkopen. Entsprechend sieht die Europäische Gesellschaft für Kardiologie in den 2004 aktualisierten Leitlinien i. d.R. keine Einschränkung der Fahreignung nach abgelaufener neurokardiogener Synkope für Gruppe-1-Fahrer (z.B. PKW, Motorrad). Symptomkontrolle wird aber bei „schweren“ (sehr häufigen oder wiederholt unvorhersehbaren oder während einer Hochrisikotätigkeit auftretenden) Synkopen als Voraussetzung für die Fahreignung für Gruppe-1-Fahrer und eine effektive Therapie als Voraussetzung für Gruppe-2-Fahrer (z. B. Bus, schwerer LKW, Taxi) angesehen.
