Erfolgreiche epilepsiechirurgische Behandlung eines Patienten mit fokaler Epilepsie und multiplen zerebralen Läsionen

Abstract

This case report describes the successful surgical treatment of a patient with focal epilepsy who had three distinct lesions in the MRI: two cavernomas, one located posterior temporal left and one mesial temporal right, and in addition a left sided hippocampal sclerosis. The decisive finding was a subtle but consistent ictal seizure pattern in the surface EEG over the left posterior temporal area. An extended lesionectomy of the posterior temporal cavernoma led to seizure freedom (follow-up 2 years). The bilateral mesial temporal structural abnormalities do not seem to be capable of producing epileptic seizures.

Zusammenfassung

In diesem Fallbericht wird eine erfolgreich epilepsiechirurgisch behandelte Patientin vorgestellt, die in der Kernspintomographie drei Läsionen aufwies: zwei Cavernome, eines links posterior temporal, eines rechts mesial temporal sowie zusätzlich eine Hippokampussklerose linksseitig. Diagnoseentscheidend war ein zwar subtiler, aber konsistenter iktaler EEG-Befund im Oberflächen-EEG mit einem Anfallsmuster über der links posterior temporalen Region. Eine erweiterte Läsionektomie des links temporal posterior gelegenen Cavernoms führte zu bislang 2-jähriger Anfallsfreiheit. Die bilateral mesial temporal gelegenen Läsionen sind offensichtlich nicht epileptisch aktiv.
