Abstract
The availability of new antiepileptic drugs (AEDs) has broadened the spectrum of medical treatment options in epilepsy. The new agents, together with the established compounds, offer substantial choice for doctors treating patients with focal or primary generalized epilepsy. In general, most of the newer AEDs are not necessarily more effective but usually better tolerated than the traditional agents mainly because of their favorable pharmacokinetic profiles and fewer interactions, which renders them, in particular, valuable for combination therapy. Because treatment options have been widened with the introduction of several new AEDs, drug therapy can now be tailored to the requirements of the individual patient. Nevertheless, significant safety and efficacy issues continue to exist and there is a strong need for the development of even better agents. This review focuses on clinical aspects of the main new AEDs, particularly on monotherapy, therapy of primary generalized seizures, teratogenicity, and recent aspects of the sideeffect profile of the newer compounds.
Zusammenfassung
Die Optionen in der medikamentösen Behandlung fokaler und generalisierter Epilepsien werden durch die neuen Antiepileptika (AE) erheblich erweitert. Im Allgemeinen sind die neuen Substanzen nicht wesentlich wirksamer als traditionelle AE, wohl aber ist das Nebenwirkungsspektrum einiger der neueren Medikamente als günstiger einzustufen, was ein entscheidender Vorteil in der häufig notwendigen Langzeittherapie sein kann. Das vielfach günstigere Nebenwirkungsprofil ergibt sich vor allem auch aus der bei den meisten Präparaten günstigeren Pharmakokinetik mit häufig im Vergleich zu den traditionellen AE geringer ausgeprägten oder sogar fehlenden Medikamenteninteraktionen, dies favorisiert sie in ihrem Einsatz bei einer Polytherapie. Aufgrund dieser Erweiterung der medikamentösen Behandlungsmöglichkeiten kann den individuellen Bedürfnissen des einzelnen Patienten besser entsprochen werden. Allerdings existieren auch weiterhin Effizienzprobleme und bei einigen Substanzen relevante Nebenwirkungen, sodass die Entwicklung optimalerer Substanzen geboten ist. Die Arbeit gibt aus klinischer Sicht einen Überblick über aktuelle Aspekte der wichtigsten neuen AE und konzentriert sich dabei auf die Themen Monotherapie, Therapie primär generalisierter Epilepsien, Teratogenität und aktuellere Aspekte zum Nebenwirkungsprofil der neuen Substanzen.
